One of the major objectives of physiotherapy treatment services to clients in their homes is to increase their mobility, safety and independence to ensure they can remain in their home for as long and as independently as possible, within safety limits.
At PhysioPro we aim to achieve these objectives by offering specialised and multidisciplinary aproach to these challenges. In this package of services, we target Fall prevention as one of our main goals.
Identification of risk is the key issue and should be done on all levels.
It does not matter if you are client, family member, carer, neighbour, Allied health professional, GP or a Medical Specialist.
If you see or hear about a person that is having problems with:
- Falls
- Reduced balance
- Reduced Mobility
- Poor vision
- Dizziness
- Difficulties with activities of daily living
- Incontinence
- Physical De-conditioning
- Progressive illness (Parkinson's disease for example)
- Returning home from/ leaving for surgery
- Living alone without sufficient assistance
This person is at high risk for falling and this risk has to be addressed by professionals.
When you get us involved, we use a problem solving approach that includes:
- Arrival at the place of residence
- Conducting a thorough interview
- Performing Comprehensive functional and Physical assessment
- performing Environmental risk assessment (limited assessment)
- Creating Personally tailored rehabilitation and maintenance programs
- Equipment prescription where needed
- Carer instruction where needed
At this stage we communicate with or involve the right professionals if necessary:
- Occupational Therapists
- Speech Therapists
- Social Workers
- Psychologists
- General Practitioners
- Medical Specialists
- Case Managers
- Governmental organisations
- Other organisations
From this stage and on, we accompany our patients as needed and operate in conjuction to other services that are involved.
